St. John’s Episcopal Hospital in Far Rockaway, New York, has received three national awards from the American Heart Association for its work in cardiovascular and diabetes care. The hospital was honored with the Get With The Guidelines–Heart Failure Gold Plus Award, the Target: Heart Failure℠ Optimal Award, and the Target: Type 2 Diabetes Honor Roll Award.

These recognitions are given to hospitals that follow up-to-date clinical guidelines and demonstrate strong results in treating patients with heart failure and Type 2 diabetes. The hospital’s leadership credits its staff for these achievements.

“Receiving these recognitions is a direct reflection of our team’s exceptional efforts,” said Donald T. Morrish, MD, MMM, Chief Executive Officer at EHS. “We are proud to deliver care that leads to a decreased recovery duration, fewer readmissions, and meaningful, lasting improvements in the lives of our patients and their families.”

The Heart Failure Gold Plus Award is awarded to hospitals that consistently apply evidence-based standards developed by the American Heart Association to improve patient outcomes.

St. John’s also received the Target: Heart FailureSM Optimal Award. This award recognizes hospitals meeting strict requirements for medication adherence, early follow-up appointments, coordination of care after discharge, and patient education. The “Optimal” designation means most eligible patients at St. John’s have received this level of care.

Additionally, St. John’s earned the Target: Type 2 Diabetes Honor Roll Award for providing current best practices in caring for hospitalized patients with Type 2 diabetes—who face higher risks for heart disease and stroke.
